Plastic Surgery has office hours in two locations, Newburyport and
Haverhill, MA. Please feel free to print out directions to our offices
and utilize this website to download your patient forms and read through
the frequently asked questions page. We hope
you'll take the time to learn more about Dr.
Sasmor, Dr.
Kutka and our staff. Please contact the
office by telephone if you need further assistance. Information for
both offices is noted below and to the right.
Office Hours: Mon-Fri 9:00-5:00
You can make an appointment or inquire about our services by calling
us during regular office hours. Patients are seen by appointment only
Monday-Friday. The receptionist is available daily to answer any questions
that you may have. Dr. Sasmor or Dr. Kutka can be reached through
our answering service if there are any problems after hours.
About our Staff
Consisting of a friendly front office staff,
2 Licensed Practical Nurses and 2 trained medical Estheticians, our
staff prides themselves on the ability to put a smile on all our patients
faces. We realize that your health is important, and part of the healthcare
process is making sure your registration, billing, insurance, and
procedural questions are answered by a knowledgeable and concerned
staff member. We also realize that practicing patient confidentiality
is of utmost concern in any healthcare environment, especially our

Insurance and Referrals
We accept more than 25 insurance plans. Please contact
our office with questions about your specific carrier. Do not hesitate
to call if you do not see Dr. Sasmor or Dr. Kutka ’s name in
your insurance brochure—we may have joined your plan recently.
We also accept Medicare, Medicaid, and Champus.
Please bring your card with
you to your first appointment.
If your plan requires you to pay a
co-payment, we ask that you pay it at the time of your visit. Most
insurance carriers now stipulate that all specialty care must be approved
by a primary care provider prior to a visit. The insurance carriers
also require that specialists be on the insurance plan’s list
of providers. To avoid problems, before making an appointment, please
be sure you have discussed any referrals with your primary care provider
and that you bring the referral information with you to every visit.

Consultation:
We believe that the consultation begins with the prospective patient's
first phone call to obtain information and to set up an appointment.
From that very moment we provide as much information as possible in
a pleasant and friendly manner. We want our patients to feel comfortable
to ask questions, make comments, and to contact us as many times as
needed.
During the initial consultation, which usually lasts for 15 - 60 minutes
depending on the nature of the consult, we discuss all issues relating
to the specific procedure of interest. A variety of educational materials
are utilized to help explain the medical procedures, benefits, risks
and possible complications. An important part of the consultation
is to establish realistic expectations. Once it is certain that the
patient is prepared for surgery, has realistic expectations, and all
of his/her questions have been answered, then surgery is scheduled.
